Writing a Requirements Document and Implementation Plan

A specifications and implementation plan will help you make a business case for your project and should be included with your submission. Here is an outline of things you need to consider when planning your project.

Grant Guidelines

Grant LOI and Application

The online Grant LOI and Application that you will be filling out is provided here in a Word Document. This allows you to prepare your responses which can then be copied and pasted into the online submission. It can also be used by your organization as a record of what you submitted to the Foundation. The document also includes some instructions for using our online application process. 

Shavlik Final Report Requirements

If your proposal was funded, you will need to download and submit this narrative report on your project and attach financial information to submit to the Foundation one year following your grant award. This allows you to be considered for future grants from the Foundation. The reports can be sent via mail or email (see contact information).
